Ophthalmic Pathology Examination
Up one levelOphthalmic Pathology Registration Form (Trainee)
Interactive Form - Click on the file link to open the form on your computer screen. Complete All highlighted fields online. Then click the "Print Form" button at the top right hand corner of the form. Complete cardholder's signature (Section C - payment by card) and Section D (Candidate signature). Fax or mail your completed form to College, together with required paperwork.
Exam Registration Form (OTS Use)
Interactive Form - Click on the file link to open the form on your computer screen. Complete All highlighted fields online. Then click the "Print Form" button at the top right hand corner of the form to print a hard copy. Complete Section F (Statement of privacy). Complete cardholder's signature in Section E if paying by card. Then fax or mail your completed form to College.
